A long, long time ago, a women named Oiwa was married to a man named lemon and they lived in Yotsuya. One day, a rich lady fell in love with lemon and captured his heart with her money. Lemon ordered his servant "Put a little of this medicine into Oiwa's every meal".
Day by day, Oiwa grew weak, lost her hair and the right side of her face became deformed. After this she was treated cruelly by the people around her and she died holding a grudge against them. After her death many strange things happened and all of the people who had mistreated her died.
民谷伊右卫门的妻子阿岩，她的父亲惨遭丈夫杀害，自己又因为妨碍到伊右卫门再婚而被下毒毒杀，虽然未死容貌却变得十分丑陋，被附近的人们称为“妖怪”。知道了一切的阿岩发誓复仇，愤怒的死去化成了真的怨灵，向伊右卫门展开报复。
